***All Student-Athletes must complete new Sports Medicine Physical Forms Book yearly and submit with all information completed to Sports Medicine.
***Physicals require Sickle Cell Blood Test and Cardiac Screening to be completed and stamped by the examining Doctor or Physicians’ Assistant.
